Propranolol is non selective beta adrenergic blocking agent and blocks both Beta-1 and Beta-2 receptors. Beta blockers lower blood pressure in hypertension, they don’t induce postural hypotension, because the alpha adrenoceptors remain functional.

Propranolol decreases cardiac output due to its negative inotropic and chronotropic effects. It directly depresses sinoauricular and atriventricular activity. Cardiac output, work and oxygen consumption are decreased by blockage of Beta1 receptors.

Beta receptors dilate blood vessels; blockage of beta receptors prevents vasodilation. The reduced cardiac output leads to decreased blood pressure. This hypotension triggers a reflex peripheral vasoconstriction that is reflected in reduced blood flow to periphery. Hence, gradual reduction of both systolic and diastolic blood pressure in hypertension patients

Reduced blood pressure causes increase in sodium retention and plasma volume. Propranolol also interferes with glucose metabolism leads to decrease glycogenolysis and decreased glucagon secretions.

Beta adrenoceptors present in lungs are also blocked due to Propranolol’s beta receptor blockage; causes bronchoconstriction thus contraindicated in Asthma patients.

Indications: 

Hypertension, Glaucoma, Migraine, Hyperthyroidism, Angina pectoris, Myocardial infarction, Anxiety, cardiac arrhythmias etc

Side Effects: 

Tiredness, lack of drive, G.I. upset, nightmares, forgetfulness, rarely hallucinations, muscle fatigue etc
